Your plan of care is developed and coordinated by our staff with input by you and direction from your physician. Any individual can be referred for Home Care Services by their physician, family, friend, or even themselves. A Minneota Home Health Care nurse will meet with you, to determine your needs and answer your questions at no cost to you.

Your care will be managed by a Registered Nurse who will be your consistent contact. She will serve as a resource person to answer your questions and arrange the assistance you need. Home Health Aides are specially trained to meet the needs of individuals in their homes.
